Basara Dental Care is pleased to offer a Dental Membership Plan. This is a comprehensive plan that will cover a wide range of dental treatments. Please read the agreement carefully and let our front desk staff know if you have any questions.
Our Membership Plan Benefits per year:
- Two Preventative Cleanings per year (in the absence of active periodontal disease)
- Two Routine Exams per year
- Annual Check-up X-rays, once per year - Bitewing, Full Mouth or Panoramic
- Two Fluoride treatments per year up until age 18
- One Emergency Exam and One Periapical (PA) X-ray per year
- 15% Discount off Fillings, Crowns, Scaling/Root Planning, Whitening Trays and Cosmetic Surgery
- 10% Discount off Sealants, NightGuards, Implants, Bridges, Extractions, Bone Graft and Membranes
Membership Plan Advantages:
- No yearly maximums
- No deductibles
- No age limitations
- No waiting periods – immediate eligibility
Yearly Maximum Cost:
- Child Membership (13 years and younger): $260
- Adult Membership (14 years and older): $385
Program Exclusives and Limitations:
- This is NOT dental insurance. This is an in-house dental membership plan and is not intended to act as or replace dental insurance. This plan is ONLY good at Basara Dental Care. It CANNOT be combined with any other dental insurance or financing programs such as CareCredit.
- Membership premiums are due in full at the time of the first exam. Payable on a yearly basis thereafter. The covered member agrees to pay all balances in full at the time of treatment.
- All payments are non-refundable. No refunds of premiums will be issued at any time if the participant decides not to utilize the plan. All financial terms for Basara Dental Care apply to the membership plan.
- Membership fees and discounts are subject to change on an annual basis.
- Services not used, may not be "rolled over".
- Memberships are not transferable.
- No additional discounts will be applied, including senior, professional, etc.
- If an appointment is cancelled without 24 hours prior notice, a cancellation fee of $50 will be applied to the patient's account.
